Annex A for manufacturing/merchandising or retail company; Annex B & B-1 for Real Estate Company
Annex C for construction Industry
Softcopies must be stored in DVD-R or USB Flash Drive with proper label which will be submitted with NOTARIZED CERTIFICATION as shown in Annex D.
